Missouri Common Grant Application - Modified [ Word doc ]
Lutheran Foundation of St. Louis is one of several area funders using the Missouri Common Grant Application. However, Lutheran Foundation has modified the Application to include additional questions of interest to our organization.
Grant Application User's Guide [ .PDF doc ]
The Grant Application User's Guide provides section-by-section instructions and examples for how to complete a strong Grant application. Also included are suggestions on developing a strong Logic Model and Evaluation Plan. The User's Guide also identifies what Grantees must track and report to the Foundation in each year of funding.
Congregation Mini Grant Application [ Word doc ]
Congregation Mini Grants are open only to Lutheran Church-Missouri Synod congregations to pilot a new social service outreach ministry. All others should complete the Missouri Common Grant Application - Modified.
This form is used by staff and the Board of Directors to review and evaluate all Grant Applications.
Grant Application Review Form [ Word file ]
Depending on the year in which your grant was awarded (not necessarily the year in which you applied), reporting requirements vary slightly. Please be sure to use the appropriate reporting forms below. Reference the “Grant Period” on your Grant Agreement to identify in which year your grant began.
Grants Awarded in 2014 and Beyond
Grant Report Form (for Interim, Annual, and Final Reports) [ Word doc ]
Complete and submit one copy of the Report Form to Lutheran Foundation. Indicate on the form if this is an Interim, Annual, or Final Report. Not sure when your reports are due? This information is listed on the first page of your Grant Agreement. Reporting forms are the same for each funding focus area.

Grants Awarded Between 2009 - 2013
Complete and submit one copy of the report appropriate to the Funding Focus Area in which the grant was awarded. Not sure which report is due when or which Focus Area you’re in? This information is listed on the first page of your Grant Agreement.
